When you apply for various pathways you may need various copies of assessments and reports. If you do not have your own copies, the Education Department will have a file that has reports from the school guidance officer, a teacher psychologist and the school speech pathologist. Or you may have to go to the hospital’s Health Centre where you had assessment by a psychologist, a speech pathologist, a physiotherapist, an occupational therapist, an audiologist, an ophthalmologist. You may have been assessed by an agency such as an Intellectual Disability Services Council, the Autism Association, Down Syndrome Society, or by private professional. You should ask that agency or professional for copies of your assessment.
