Access Equipment
Specialised equipment is available for loan to enable students with physical disabilities to participate in preschool/school.
The range of equipment provides for:
Seating
Transfer and Standing
Toileting / Self Care
Mobility
Classroom Access.

Guidelines for Provision
Schools requesting equipment:
Obtain a letter from a Health Professional to support the application for Access Equipment
Download a Request for Access Equipment Form
Forward the request form with the Health Professional recommendation to SERU.
SERU will forward a loan agreement prior to delivery. The Principal/Delegate is required to sign the agreement and return to SERU. The equipment will be delivered to the school by courier.
The school may need to contact the therapist when the equipment arrives. Equipment must be used as described in the child/student Transfer and Positioning Care Plan or as recommended by the therapist.

What is a Loan Agreement?
Access equipment is available for loan for use by students within DECS sites.
It remains the property of SERU. It is not to be transferred between students, and must
be returned to SERU when the student no longer requires it.
The loan agreement is sent to school sites prior to dispatching the equipment.
The agreement details loan terms and includes the schools responsibilities.
Damaged, lost or stolen equipment must be replaced through an insurance claim
by the relevant preschool or school
Access equipment will only be dispatched after the relevant loan agreement/s
have been signed and returned to SERU.
How do we go about modifications or repairs?
Contact SERU if equipment requires repair.
If modifications are required to equipment on loan a new Request for
Access Equipment
form and letter from the health professional must be sent to SERU.
What happens when a child changes schools?
SERU must be notified when a child student transfers to another school.
Loan agreements will then be sent to the new school.
School staff contact the DECS courier to arrange transfer of the equipment to the new school.
If a child/student leaves school, the school must return the equipment to SERU.
What happens when a child leaves the school?
When a child leaves the DECS site the equipment must be returned to SERU immediately.
What are the safety requirements for sites on electric equipment borrowed from SERU?
All equipment is regularly tagged by SERU
It is the site's responsibility to ensure that tagging is carried out on equipment while on
long term loan
SERU has a service contract with Huntleigh Healthcare for the servicing of
change tables and hoists.
Country sites are contacted by Huntleigh Healthcare to arrange a time to carry out the service at the school.
